01-OBPR-01 Electronic Forms Submission Interface
Thank you for using the new Electronic Forms Submission Interface to streamline the processing and review of your proposal. By using this interface you will:
- Receive immediate confirmation that your Forms have been received
- Reduce the amount of data entry required to process your forms
- Reduce the possibility of errors occurring during data entry
- Help to streamline the handling of your proposal through the review, selection, and procurement process.
Please note: We must still receive original + 20 printed copies of your proposal and forms, including a signed copy of Form A, at our office by the submission deadline of April 26, 2001.
Use of the Electronic Forms Submission Interface is optional and does not change the basic submission procedure, but does allow for quicker processing of your proposal once the hard copy version is received at our office.

MINIMUM WEB BROWSER REQUIREMENTS: You must have at least NETSCAPE 2.0 or INTERNET EXPLORER 4.0 to use this interface.
Steps to follow: (EACH STEP MUST BE COMPLETED IN THE ORDER SHOWN)
- Download the file containing Forms B-to-H saved as an RTF file: FormsBtoH.rtf (275 KB)
- Using Word, Wordperfect, or another RTF compliant word processor, fill out FormsBtoH.rtf and save the file to your hard disk. Note where the file is saved.
- Return to this page.
- Fill out the
Electronic Form A on-line. When you fill out this form, you will be asked to upload the copy of FormsBtoH.rtf that you filled out in step 2.
- NOTE:This interface uses SSL encryption for security. If you receive an error message that the Certificate Authority has expired, please see
the Thawte web site for
a quick procedure to update the security information on your browser.

- If you have difficulty in correcting this problem,
you may choose to use the
unencrypted version of Electronic Form A
- After submitting the Electronic Form A in step 4, you will immediately receive a copy of Form A downloaded to your computer as an RTF file. Save this file in the same directory or folder where you saved FormsBtoH.rtf
- Print out FormA.rtf and FormsBtoH.rtf
- Have Form A signed as specified.
- Make 20 copies of the your Proposal, including the signed Form A and Forms B-to-H, and send them along with the original to our office at
